The Block Fee Program is an optional membership. It bundles the cost of common uninsured services that OHIP does not pay for, such as medical forms, notes, and prescription renewals by phone or fax. Joining is completely your choice. You do not need a plan to be a patient here, and you do not need one to book or receive insured care. Insured appointments can always be booked by phone at no charge.

Form fee policy/disclaimer:
If a membership is cancelled or expires, and the cost of a medical form completed by your healthcare provider during the membership period exceeds the membership fees paid, the member is responsible for paying the difference for the forms.

Please note: Patients of specialists are not eligible for Block Fee Programs (Dr. Goulet and Dr. Ekong)
